Do I need a referral from my physician?

No, you do not need a referral from your physician to be seen by a physical therapist in the state of North Dakota. A physical therapist can evaluate and treat without a prescription from a physician. You can contact me directly to schedule an initial assessment. Should your concerns require more than physical therapy, an appropriate referral to a medical provider will be made. We do ask that you provide me the name and contact information of your primary care physician, OBGYN, or midwife on the intake form.

What can I expect at my first visit?

At your first visit we will listen to your story and concerns, and ask additional questions to get a full picture of what you are experiencing. We will then perform a thorough evaluation and examination, provide a description of what is contributing to your symptoms/limitations, and we will collaborate to develop a plan of care specific to you. You will leave with self-treatment instructions, a program of therapeutic exercises, and injury-specific information as it relates to activity modification, ergonomics, or healing timelines. Together, we will also determine a frequency and duration of treatment based on your expected outcomes and personal goals.

How do I schedule an appointment?

Please provide a brief description of what you wish to be seen for, and the general day and time that works best for you. If you have additional questions or would like to speak over the phone prior to scheduling, we can set up a time for a phone call.

What forms do I need to fill out prior to my appointment?

We will send you a link to the online intake forms once you are scheduled. These can be completed online, and will need to be completed prior to your first appointment. If you prefer to fill these out by hand at your appointment, paper copies will be provided for you.

How do I pay for my appointment?

Move Wise is a different kind of practice. We are here to serve you, put you first, and personalize your care. We are a cash practice, meaning your insurance is not billed directly. The reason for this is to create an atmosphere that allows me to serve you with your best interests in mind. One-on-one sessions, treatment is personalized, no cookie-cutter rehab programs; my goal is to get you better faster.

If you wish, a physical therapy invoice can be provided upon request for you to submit to your insurance for potential reimbursement. You submit the invoice to your insurance company, and they will reimburse you directly if applicable.
